Surgical Procedures: Financial Responsibility Agreement

Thank you for choosing County Obstetrics and Gynecology for your care.

We understand that preparing for surgery can be stressful, and we are committed to making the financial aspect of your care as transparent and manageable as possible.

Your surgical care includes the procedure itself and any postoperative visits. 

Additional services—such as:

  • Facility fees
  • Anesthesia Fee
  • Pathology Charge
  • Imaging Fee

…are billed separately by the provider or facility performing them and may result in additional financial responsibility depending on your insurance coverage.

We will discuss any anticipated additional services with you so there are no surprises.

We know that health insurance can sometimes be confusing, and our goal is to make the process as clear as possible for you.

Before your surgery or procedure, our office will review the details of your insurance plan and, when required, obtain any necessary preauthorization or prior authorization (PA) from your insurance company.

Our billing team will prepare a personalized estimate based on the benefits information we receive from your insurance company.

As part of this process, we will provide you with an estimate of your financial responsibility, including your

  • Individual Deductible
  • Family Deductible
  • Co-Insurance
  • Co-Payments

Please note that this is only an estimate and your final financial responsibility may change based on how your insurance company processes the claim.

This helps us provide you with a clear understanding of your anticipated financial responsibility and work together to create a payment plan that is simple and manageable.

If your deductible has not been met, we will require payment of your estimated deductible and/or coinsurance prior to the procedure.

Why We Collect Your Deductible Early

Like many practices across the country, our policy is to collect this portion of your responsibility prior to your surgery date.

Many surgical services apply toward your deductible. Collecting your estimated deductible or co-insurance early helps prevent surprise bills later and ensures that financial matters are settled before your procedure, allowing us to focus entirely on your care.

Check – No extra charge
Debit card – No extra charge
ACH transfer (electronic bank payment) – No extra charge
Cash – No extra charge
💳 Credit card – A 2.6% processing fee applies. This fee is charged by the credit card company and is non-refundable.

Refunds

After your claims are submitted, your insurance company may take several weeks to process them due to their documentation and administrative requirements. If a refund is due, our office will issue it to you promptly once processing is complete.

If your insurance pays and you are due a refund:

  • Payments made by check, debit, ACH, or cash will be refunded in full.

  • Payments made by credit card will be refunded minus the 2.6% processing fee.
